Health Services

The health and social services department coordinates and provides health and social services to promote a healthy school community and maximize educational outcomes.

Health Requirements for School Enrollment

All students need proof of immunizations or a completed certificate of exemption for immunizations, on file, the first day of school attendance. If you need to update this information, please upload the documentation through PARENT VUE or connect with your school office. If your student plans to attend, the following forms must be on file at your student’s school:

  1. Medically verified CIS form OR
  2. Certificate of Exemption

Students who need medications or treatments during school, are required to have a signed medication authorization form and necessary supplies from a licensed healthcare provider before we can administer medications/treatments. A student may not enroll in school without the ordered medication and completed medication authorization form for required medications or treatments.

*Students that are eligible for the McKinney-Vento program are not required to provide health documents at the time of enrollment as a condition of enrollment. If there are barriers for the family to access these documents please contact the McKinney-Vento liaison.

Latest Immunization Updates

New Immunization update for upcoming school year

Students entering school must get the required doses listed in the chart linked here School Immunization Chart. School aged students (Preschool thru 12th grade) must meet the immunization requirements for their grade in school.  School aged students who are in programs and sports at school sites must meet immunization requirements to participate. The requirements follow the national Advisory Committee on Immunization Practices (ACIP) Immunization Schedule with requirements specified by grade level.

Tdap immunization update 

For the school year all students attending grades 7 through 12 must have had one dose of Tdap vaccine:

  • For students in 7th - 11th grades, the student must have a Tdap at age 10 or greater to be accepted for school entry.
  • For students in 12th grade, the student can meet the Tdap requirement at age 7 or greater for school entry acceptance.
MMR Immunization Exemption Law Change 

As of July 28, 2019, a new state law (House Bill 1638) removes the personal and philosophical option to exempt children from the measles, mumps and rubella (MMR) vaccine required for school and child care attendance.

Children without a medical or religious exemption, current MMR vaccines or proof of immunity will need two doses of MMR vaccine to be allowed to start school.

Attendance Matters

Students can suffer academically if they miss 10% of the school year-- that's just one day every two weeks.
